Service Inquiry into the loss of F-35B Lightning ZM152
IFA Comment: There are lessons for the civil aviation world. The importance of preventing Foreign Object Debris (FOD) is universal. ‘REMOVE BEFORE FLIGHT’ components are often referred to as “red tag items”. Engineers and pilots need to ensure a robust procedures are in place in order to verify the location of these items at all times.
A simple mistake like leaving protective covers on a tube or engine can cause delays or, in the worst cases, a fatal accident.
A Service Inquiry has been completed into the loss of F-35B Lightning ZM152 (BK-18) of 617 Squadron RAF, embarked on HMS QUEEN ELIZABETH on 17 November 2021.
The final report has been published to inform the military Chain of Command and the public of the findings of the inquiry and its recommendations to improve defence safety.
